🔵🇰🇷Meniscocapsular Separations

  • Vertical tears located just peripheral to the meniscus act similarly to soft tissue injuries, potentially causing instability in the meniscus.
  • These kinds of injuries are known as meniscocapsular separations, which occur when the meniscus detaches from the capsule.
  • More frequently observed on the medial side, meniscocapsular separations can take place at the coronary ligaments, at the junction where the meniscus meets the capsule, or in the meniscus's outer part.
  • Similar to red zone tears, these separations tend to have a higher likelihood of healing on their own.

Radiol Clin N Am 51 (2013) 371–391

🔵🇰🇷Indicators of a medial meniscocapsular separation include:

  • The meniscus is displaced in relation to the tibia.
  • Presence of fluid in the space between the meniscus and the capsule.
  • A tear is located in the outer area of the meniscus.
  • An uneven edge of the meniscus.
  • Additionally, a tear often occurs in the meniscotibial (coronary) or meniscofemoral ligaments, usually at their point of attachment to the meniscus.

🔵🇰🇷Meniscocapsular separation vs ramp lesion

  • Ramp lesions represent a distinct subset of meniscocapsular injuries, typically linked with knees lacking an intact anterior cruciate ligament (ACL).
  • In the literature, the phrases 'meniscocapsular separation' and 'ramp lesion' have often been used interchangeably.
  • However, it is now believed that ramp lesions constitute just a portion of what is encompassed by meniscocapsular separation.

Radiol Med. 2021 Aug;126(8):1106-1116.

🔵🇰🇷A misdiagnosis of meniscocapsular separation can occur due to several factors:

  • Bursitis of the medial collateral ligament (MCL).
  • The presence of a cyst in the meniscus.
  • A tear in the MCL.
  • An accumulation of fluid within the joint.
  • Fat around the periphery of the meniscus.
  • Fluid around the meniscus in a knee that has undergone surgery.
